"I want to look younger, but not plastic."

When patients initially come in, a common request we hear is, “I want to look younger, but not plastic”.  Our immediate response is “Ultherapy!”
Usually with an enticing look on their face, they say “what is that?”…
Ultherapy or Ulthera will tone loose or wrinkled skin by using the body’s own healing response. Ulthera will tighten the neck and jowls or lift a sagging brow, for example, which in turn reduces the excess eyelid skin, giving the eyes a more refreshed appearance.  It requires no special preparation and results typically appear after one treatment. The ultimate lifting will take two to three months, as rejuvenated, stronger collagen replaces the existing tired collagen in your skin.
To get a little bit more detailed, I’ve included a few frequently asked questions about Ultherapy:

What is Ultherapy?

Ultherapy is a non-surgical face treatment that uses ultrasound to actually lift and tone loose or sagging skin without any downtime by using the body’s own healing response.


What is so different about this procedure?

Ultherapy is the only non-invasive treatment that can specifically target the deep foundation below the skin that is addressed in cosmetic surgery without cutting or disrupting the surface of the skin. It is also the only procedure to use ultrasound imaging, which allows us to actually see the layers of tissue we target during treatment and ensure the energy is deposited precisely to where it will be most effective.

How does Ulthera work?

Ultherapy deposits focused ultrasound energy below the surface of the skin to stimulate the body’s creation of new collagen. As a result, skin will actually begin to lift and tone over time.

What does collagen do?

Collagen is a natural protein that gives skin its youthfulness by keeping it firmed, toned and elastic. As we age, collagen loses its elasticity and its ability to stand up to the effects of gravity that pull the skin downward. Ultherapy jump starts a repair process that produces fresh, new collagen.

Is Ultherapy surgery?

No. Unlike a face lift, Ultherapy is a non-invasive procedure that addresses the skin and support layers below it, but doesn’t entail cutting or disrupting the surface of the skin.

Can Ultherapy replace a face lift?

Ultherapy is an “uplift” not a “face lift.” While it is not a replacement for surgery, it is a viable option for those not ready for a face lift or those looking to prolong the effects of cosmetic surgery.

How is Ultherapy different from lasers?

Lasers typically address issues in the superficial layers of the skin (e.g. fine lines, wrinkles, pigment changes). Ultherapy addresses the deep skin layer and the foundational layer addressed in cosmetic surgery that lift and lend support to the skin. Therefore, the two technologies are quite complementary!

Is Ultherapy safe?

The procedure has been cleared by the FDA after demonstrating safety in clinical studies, and tens of thousands of treatments have been performed safely worldwide. In addition, ultrasound energy has a proven track record, with use in the field of medicine for more than 50 years.

Who is a good candidate for Ultherapy?

A good candidate for Ultherapy is someone with skin that has “relaxed” to the point of looking, and often feeling, less firm. A lowered eyebrow line or sagging skin on the eyelids, for instance, is often the first sign of “maturing” skin. Typically, those in their thirties and older who have mild to moderate skin laxity are candidates.
While Ultherapy is not a replacement for a surgical face lift, there are many people who want some lifting but are not ready for surgery, either mentally, financially or logistically. There are also younger people who want to “stay ahead of the game” as well as those looking to prolong the effects of cosmetic surgery.

How long does the ultrasound treatment take?

A full face treatment takes 45-60 minutes, and a partial face or brow lift treatment can take around 30 minutes–neither with any downtime. This is why it’s been nick-named “The lunch-time face-lift”.

What problem areas does Ultherapy target? What kind of results do you get?

Ultherapy addresses mild to moderate loose or sagging skin. Specifically, by reconditioning and renewing the skin from within, Ultherapy will lift the brow, which in turn reduces excess skin on the lids, opens up the eyes, and gives a more refreshed look overall.



How long until I see results?

Patients may enjoy some initial effect but the ultimate lifting and toning will take place over 2-3 months, as tired collagen is replaced with new, more elastic collagen. As this collagen-building process continues, further improvements can appear up to 6 months following a procedure.

How long do results last?

Patients treated with Ultherapy still have fresh young collagen after a year’s time following the procedure, but skin continues to age. Future touch-up treatments can help keep pace with the aging process, which varies by individual.

Will I need more than one treatment?

The majority of patients only need one treatment; however, some may benefit from more than one treatment (depending on how much laxity they have and their body’s own biological response to the ultrasound and collagen-building process).

Is there any downtime?

Ultherapy is an entirely non-invasive procedure and as such, there is no downtime. You are able to return to your normal activities right away, and there are no special measures you need to take.

Will I feel the ultrasound during the treatment?

Yes. What you are feeling during the treatment is actually tiny amounts of focused ultrasound energy being deposited below the surface of the skin.

What does it feel like?

Comfort thresholds vary from patient to patient, but there can be some discomfort while the ultrasound energy is being delivered. This is temporary and a positive signal that the collagen-building process has been initiated.
Your practitioner will consult with you prior to your skin treatment to make your experience as pleasant as possible. Patients typically leave comfortable and excited about the benefits to come!

Are there any side effects?

There may be slight redness for up to an hour or so following the treatment, and a small percentage of patients may have slight swelling, tingling or tenderness to touch, but these are mild and temporary in nature. There is always the possibility for other less common post-procedural effects, which your practitioner will review with you.

How much does it cost?

When is the Best Time to Use Retinol Products?

What You Should Know About Wrinkle Fillers

Injectable wrinkle fillers can give you a more youthful look for a fraction of what a traditional facelift costs. Most will fill lines and wrinkles in less than 30 minutes with results that can last from 4 months to more than a year.

Injectable wrinkle fillers, unlike Botox injections that relax the muscle under a wrinkle, fill the line, crease, or area with one of several different substances. As a result, trouble spots nearly disappear.


Wrinkle fillers can also be used as "volumizers," plumping and lifting cheeks, jawlines, and temples; filling out thin lips, and plumping sagging hands.

The treatment is fast and easy. But all wrinkle fillers have a downside, including the risk of allergic reaction and the formation of tiny bumps under the skin. In some cases, those bumps may be permanent. And sometimes, a bluish skin discoloration known as the Tyndall effect happens. The color change can last for several months, but there are treatments available. In very rare cases, skin cells may die if the wrinkle fillers are not used properly. There have also been a few reported cases of blindness and nerve paralysis. Typically, the wrinkle fillers that last longer are the ones more likely to cause side effects.

Not every wrinkle-filler is right for every type of wrinkle. The least risks and best results come from using the right one correctly. That's why you should only have fillers injected by a board-certified  plastic surgeon with ongoing, special training.

Hyaluronic Acid Wrinkle Fillers

 
The most popular category of wrinkle fillers is hyaluronic acid. Each type works in a slightly different way with varying results.
 
Side effects are rare but can include redness, swelling, and bruising at the injection site. The filler may also show up under the skin as tiny bumps. This is a problem that often improves over time.
How long the results last varies from several months to over a year or two. Some research shows that repeated injections may help stimulate the body's own natural production of collagen. That will help reduce the number of lines and wrinkles. There is also some evidence that less filler is needed over time to achieve the same look.

Synthetic Wrinkle Fillers

This smaller category of wrinkle fillers includes lab-made substances that are not related to anything found naturally in the skin.

All the fillers in this group have similar side effects, such as redness, swelling, or bruising at the site of the injection. Other side effects include nodules or bumps under the skin that can be seen and felt and that, in rare instances, may require surgery to remove.

The benefits include a longer-lasting effect. And at least one filler offers permanent filling of lines and creases. Remember, products with longer-lasting effects are more likely to cause side effects. And when not used correctly, synthetic wrinkle fillers may cause disfigurement.

Synthetic wrinkle fillers include:
  • Radiesse
  • Sculptra
    Silicone

    Collagen Wrinkle Fillers

    Scientists made the first wrinkle fillers from a purified form of collagen extracted mostly from cows. Although it worked well and offered a natural-looking fill, the results didn't last long. Most collagen injections began to break down as early as 1 month after treatment. Because these wrinkle fillers were made from an animal source, they also had a higher rate of allergic reaction and required allergy testing beforehand.New ways of processing the collagen have helped lower the risks. Plus, new forms of synthetic collagens are making these injections safer and more useful to a wider range of people. Although the results don't last as long as other wrinkle fillers, many believe the results are more natural looking.Side effects of collagen injections include some risk of allergic reaction (mostly for those still using cow sources), as well as bruising and redness at the site of the injection. Collagen injections include:
    • ArteFill
    • Cosmoderm
    • Evolence
    • Fibrel
    • Zyderm
    • Zyplast

    Autologous Wrinkle Fillers

    Fat is the most commonly used substance in this category. Your own fat is surgically removed from your thighs, buttocks, or stomach, treated, then injected. You will need to have two procedures (one to remove the fat and one to inject it). Both procedures can be done in one visit. Additional fat purification steps done in the lab can be costly and time-consuming. Results can be semi-permanent, although you may need a series of injections done over time.
    Platelet-rich plasma injections (“vampire lift”) are another type of autologous wrinkle filler/volumizer. Blood is drawn from the arm, treated, then injected into the face. The effects can last 12 to 18 months.
    Risks are similar to other wrinkle fillers, including bruising, redness, and swelling at the site of the injection. Because the fillers come from your body, these injections do not require FDA approval.

    Minimizing Risks and Increasing Good Outcomes for All Wrinkle Fillers

    Wrinkle fillers are among the safest cosmetic procedures in use today. But there are things you can do to help ensure your treatment is safe:
    • Don't let price be your guide. If you are offered a wrinkle filler treatment that costs far less than the standard treatment, it's likely some compromises are being made, either in the skill of the provider or the quality of the product. Never risk making a bargain with your face.
    • All wrinkle fillers should be done in a medical setting with sterile instruments.
    • Do not get injectable wrinkle fillers from sources outside a doctor's office. Know what you are being injected with, and ask your doctor if an FDA-approved wrinkle filler is being used and if it was purchased directly from the maker. There have been reports of everything from industrial-grade silicone to baby oil being used. If a provider won't give you this information, don't let them do the procedure.
    • Use sunscreen daily to help preserve the filler and help protect against post-inflammatory pigment changes due to the needle sticks from injections.

    Is It Normal To Be Itchy After Tattoo Removal?

    Laser treatments are definitely the way to go for tattoo removal, but there are a few uncomfortable side effects you may have to deal with while the treated area heals. One of the biggest complaints we hear is that the skin in and around the treated area is extremely itchy after treatment.

    The laser tends to cause some dehydration to the skin. This in and of itself can be quite itchy. Add that to the fact that the healing process can cause some itching too, and you’ve got the potential for some very itchy skin. In most cases the itchiness is not too bothersome. We recommend a moisturizer, preferably SkinCeuticals Epidermal Repair to rehydrate the skin and relieve the itch.

    When you do apply moisturizer, do so with a deep massaging action. This will not only help the moisturizer to penetrate deeper into the skin, it will also increase the blood flow to the area which will in turn aid in the healing process and allow the immune system to flush away shattered ink more quickly. We recommend that all of our tattoo removal patients do this at least once daily after the area begins to heal, but especially if your tattoo is in an area with reduced blood flow such as the hands or the feet.

    How Many Treatments Will It Take To Remove My Tattoo?



    When it comes to estimating the number of treatments a tattoo may need in order to remove it, there are many factors that contribute to that number. Many of these factors are not readily visible. Some determining factors include, but are not limited to, tattoo color, skin color, density of ink, depth of the ink in the skin, and the patient’s own immune system.

    Busting the Botox Myths

    We’ve all heard them. Tell a friend you’re planning on getting Botox and they might list off rumors they have heard on this or that website, or a horror story from the depths of a gossip magazine. Before you get your procedure, it’s important to know the truth and fiction about this popular procedure.

    1. The number one myth you will hear is that Botox leaves you expressionless. There’s no shortage of punchlines in movies where a recent Botox patient is mocked for being unable to smile or do much more than blink, but that’s far from the truth. Injecting too much Botox can have an effect on one’s facial expression, but this is an extreme case scenario. Regular Botox procedures require injecting very small amounts of the solution into specific facial muscles–not indiscriminate application that minimizes expression. Applications by qualified, professional board certified plastic surgeons leave patients with a natural look free of unwanted lines and wrinkles, without sacrificing expression.

    2. Botox is for the rich and famous.     Is Picosure REALLY The Latest and Greatest in Laser Tattoo Removal?



    There’s been a lot of hype lately with regards to the newest FDA approved tattoo removal laser, Picosure. Its’ been seen on news publications across the country; states are happy to announce its arrival to one of their cities; and a quick Google search for “laser tattoo removal” will likely result in numerous listings devoted to the Picosure laser. So what’s all the fuss? Is it in fact the biggest breakthrough in laser tattoo removal in decades? Perhaps in some regards, but not to the extent the media hype would lead you to believe.

    The biggest difference between the Picosure laser and the more widely used Q-switched lasers like our Revlite SI is the pulse-width (the amount of time the laser’s energy is applied with each pulse). A Q-switched laser has a pulse-width of 1 nanosecond. As its name would suggest, the Picosure has a pulse-width of 1 picosecond, one thousand times smaller than a nanosecond.

    What does a shortened pulse-width mean for laser tattoo removal? In theory, a shorter pulse-width would mean less pain during treatment and less temporary tissue damage therefore a shorter healing time. After performing a Google search for “Picosure reviews” we found www.realself.com/picosure.

    There are some very detailed accounts including pictures chronicling patients’ journey through picosure treatments. Most of the pictures and descriptions of the healing process as well as many testimonials of the discomfort felt during treatment seem very closely related to what can be expected with treatment from our Revlite SI Laser.

    Picosure’s creators’ accounts of how effective their laser is at removing tattoos, leads many to believe that it can remove a tattoo in as few as one treatment and if not just one, certainly far less than a Q-switched laser. Further review of patients’ accounts of their treatments showed that multiple treatments were needed in many cases and unless the tattoo is full of blue, green, or purple inks, the Picosure is not the only laser used to treat the tattoo. Reds and yellows are especially resistant to the Picosure. This is because the Picosure laser only has one wavelength, 755nm (Alexandrite). Sadly not all colors will respond to the same wavelength. Black responds to most wavelengths so the Picosure does a pretty good job of fading black inks, but it is most effective against blues, greens, and purples.

    If a patient has reds, yellows, and oranges in their tattoo the Picosure is not the right laser for them though. Another laser will need to be used to effectively fade those colors, and what will the proponents of the Picosure use instead? Yep, you guessed it, they’ll be forced to go right back to a Q-switched laser like the Revlite SI we use here at Chic Esthetiq Med Spa & Laser because for reds, yellows, and oranges you need a 532nm wavelength.

    Here's the clincher: with the cost of Picosure laser treatments nearly double the price of Q-switched laser treatments, is it really worth it to go with the Picosure? That is totally up to the consumer, however if given a choice, I'd lean towards going somewhere that could use one laser to deal with ALL the colors that need to come out. That just makes more sense. Upon looking at the long track record of efficacy and documented safety and results from the Q-switched Revlite SI laser, well...why re-invent the wheel? Go with what has been proven to work.
